THE EFFECTS OF COMMUNICATION FAILURE ON NURSING ERROR AND PATIENT SAFETY IN SELECTED HOSPITAL SETTINGS IN BATANGAS PROVINCE

Abstract
Patient safety in healthcare is often threatened by nursing errors and communication failures, which compromise care quality and increase risks of adverse outcomes. This study examined the effects of communication failures on nursing errors among staff nurses in selected hospitals in Batangas Province. Using a quantitative descriptive-correlational design, data were gathered from 66 staff nurses through a self-structured, expert-validated questionnaire. The tool measured demographic and work-related characteristics, types of nursing errors, and causes of communication breakdowns. Descriptive statistics summarized the results, while Pearson’s correlation tested relationships at a significance level of p < 0.05. Findings showed that nursing errors such as documentation errors (M = 2.91), communication failures (M = 2.83), and failure to monitor (M = 2.78) occur “sometimes” in clinical practice. High workload (M = 3.06) and stress and fatigue (M = 3.00) emerged as the leading causes of communication failure. Educational attainment showed a significant relationship with documentation errors (p = 0.01) and failure to monitor (p = 0.05). The study concludes that communication breakdowns remain a recurring barrier to safe practice, emphasizing the need for standardized communication protocols, continuous training, and organizational support to strengthen collaboration and reduce preventable nursing errors.
