Abstract
The most important step to ensure sustainable healthcare delivery is to support community health initiatives. However, several factors make it hard for healthcare workers and residents to take part in these initiatives, which reduces their effectiveness. This quantitative-correlational research study looked into the challenges community nurses from the City Health Office faced when implementing these initiatives and examined participation among residents in selected barangays in Batangas City, including Barangay Wawa, Cuta, Malitam, and Sta. Clara. This included 60 residents, with 15 from each barangay, and 25 community nurses from the City Health Office with a total of 85 participants. The researchers made use of validated surveys designed for each group, the results then are analyzed using non-parametric tests, including the Mann-Whitney U and Kruskal-Wallis H tests, as well as frequency distribution, percentage, weighted mean, and standard deviation. The findings revealed that community nurses frequently dealt with a lack of funding, staff shortages, and insufficient resources, which made it challenging to implement the community health initiatives effectively. On the other hand, residents reported that their participation was affected by cultural or religious beliefs, limited access to resources, and gaps in health literacy. Statistical testing confirmed significant differences between the responses of the two groups. These results indicate that increasing community involvement, fostering better cooperation with local authorities, and providing sufficient support for healthcare professionals are essential to tackle systemic and community-level issues. The study offers insights into the factors that contribute to the sustainability of health initiatives and suggests strategies to improve participation, collaboration, and fair healthcare delivery at the barangay
